As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral, and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.
Responsibilities:
- Complete the nursing observations report findings to the registered nurse
- Review lab and test results, reporting critical values to RNs and providers
- Medication management including ordering medications and obtaining prior authorizations
- Dispense medications
- Communicate with medical director/providers regarding the observation in the client’s health/behavior
- Provide client and family education regarding medications and treatments
- Collaborate with our staff regarding concerns and behaviors
- Participate in Interdisciplinary Medication Review meetings
- Nursing documentation using EHR
- Update discharge planning needs
- Other duties as assigned within the scope of practice and based on agency needs
Requirements and Education:
- LPN Licensed in New York State
- 1 to 2 years of experience
- Experience working with youth and adolescent populations desired

The Behavioral Health program and the ACT program are seeking a Psychiatric Nurse Practitionerout of our Albany and Schenectady, NY Location. This position is responsible for providing comprehensive Psychiatric services to Adults who are enrolled in the ACT program. This position is full-time, 40 hours per week. Must be able to seechildren.
RESPONSIBILITIES:
- Provide comprehensive psychiatric services to adults enrolled in ACT and BehavioralHealthas assigned
- Provide consultation to staff on diagnostic and treatment questions
- Participate in formal and informal training and education to staff·Prescribe medications, monitor, and review of drug regimen for ACT enrolled individuals
- Participate in quality assurance activities, consult with the ACT Team in developing and reviewing service plans for all ACT enrolled individuals
- Provide clinical expertise, consultation, and technical assistance to the ACT Team/Behavioral Health, cooperate with the County and participate in any quality audit and utilization review ·Provide on-site/community based/home visits medication evaluations and reviews.
EDUCATION and/or EXPERIENCE
- Requires Board Certification by the American Board of Psychiatry and Neurology. Must have a minimum of two years working with adults who have Seriously and persistently mentally ill.
- Must be fully licensed Nurse Practitionerin the State of New York.
